Click on the form you need , and save it to you desktop. fill it out and email back to : sfhl@sahofhockey.org
You must have adobe acrobat to download and read the team application. It may take a few seconds.
If you use outlook or outlook express as your email program, open it first before you start to fill out the form, you will be able to send it directly to the league .
If you use a web based email program you will have save the completed form, just follow the instructions.
Each Team is required to submit the roster upload form.
You will need to save the file to your computer to fill it in. Right click on the form below, and "save target as" to your computer.
Open the form up fill it in following the instructions and attach it to an email and sent to sfhl@sahofhockey.org
Please be sure to "Save As", your team name and division, and place your team name in the subject area.
